Summary
After an image is reverted, our version of MediaWiki doesn't regenerate the thumbnails correctly. This can cause problems when images are vandalized.
Today the File Picker Meme article was vandalized after being posted on Hacker News. This is an screenshot of what happened when I tried to re-upload an image to correct its still-vandalized thumbnail.
